What to Bring to Your First Visit

On the day of your appointment, please bring:

  1. Copies of your medical history, x-rays, and/or lab reports if you have them.
  2. A list of medications that you are currently taking and their dosages.
  3. Proof of income* - e.g. tax return, pay stub, letter from employer, social security paperwork.


    *Note: This information is used to screen patients for Medicaid/Family Health Plus and for programs that offer discounted medications. It may also be used to determine whether your income qualifies you for treatment at the WCCC. Otherwise, this information does not affect your healthcare or the other services available. If a family member or friend supports you, then please bring a copy of their proof of income.

  4. Any questions you may have for the doctor.

Cancellation Policy

The WCCC has a waiting list of several weeks for new appointments. To ensure that we operate at maximum efficiency, patients must notify the WCCC of cancellation at least 24 hours prior to their appointment.
