Scheduling a Follow-Up Appointment

If you are already a patient at WCCC and need to schedule a follow-up visit, please use our appointment request form.

Before your follow-up visit, please download and complete our patient registration forms: available in English and Spanish. Please bring the completed forms with you to the follow-up visit.
