To report an incident that occurs in a clinical space please use MERS Incident Reporting
General Liability
If an incident occurs in a clinical space please report it to Patient Services at (212) 746-4293
In other spaces such as Olin, Lasdon House, etc complete either the incident report at the security desk or the report found here.
