Please fill out the below enrollment. From the dropdown menu in box 3, choose Option 1 (full blanket coverage) or Option 2 (partial blanket coverage). Please note you must choose either Option 1 or 2 before you can submit the form and send it to Risk Management and Insurance. Alternatively, the form can be printed and faxed to Risk Management at 212-746-6661 or sent via interoffice mail to Risk Management, Olin Hall, Suite 201.
