Annual Survey of External Financial Interests and External Time Commitments Survey - 2012-2013
The WCMC Conflicts of Interest and Commitment Policy requires all WCMC faculty and staff to report information about potential conflicts of interest at least annually.
The 2012-2013 Annual Survey is now available and can be accessed through the new Conflicts Management System. The Annual Survey is no longer a two-part survey. The new survey asks three initial questions. One determines whether or not you have any financial interests which need to be disclosed and the second determines whether or not you are involved in any research at WCMC. If you do not have any financial interests to disclose and you are not involved in research at WCMC, then you will not need to answer any additional questions. If you have financial interests to disclose you will be required to answer the first three questions plus an additional set of questions for each entity with which you have a financial interest. For example, if you have financial interests in three different entities, you will be required to answer the additional set of questions for each of those entities. The third question is for NIH funded researchers only and is related to any travel related to your institutional responsibilities.
If you are involved in any research at WCMC your 2012-2013 Annual Survey must be submitted before you will be permitted to complete a Study Specific Report (formerly the Study Specific Financial Disclosure form). As of April 1, 2010 the Study Specific Report that you submit with all Grant applications, Clinical Trial applications, IRB applications, and IACUC applications will be the Study Specific Report that is generated directly from the new system.
If your external financial interests change during the academic year, you must update your survey accordingly. The amendment process is completed by accessing the Conflicts Management System and updating the "Active" Annual Survey.
