Event Services Office - Request Other Services

Fill out the form below to submit an professional services request. An ESO staff member will respond within three business days to confirm any additional details and provide a quote for your services.

Professional services are only provided to affiliated WCMC or NYP staff, faculty, or students. Please note that availability and turnaround time for professional services can vary, depending on scheduled events and staff availability.

Please note that event bookings cannot be completed through this form. Any event requests sent through this form will be rejected without comment. Please use the Event Request Form for all event requests.

Service Request

The contact person will be responsible for interfacing with the ESO for all event details, as well as billing details.


Failure to submit a valid account number will cause your request to be rejected without comment.
